We explore the use of self-tracking data and their visualizations in the context of patient-provider encounters for chronic care management. We focus on endometriosis, a condition with numerous challenges for successful management. Structured interviews with 10 providers across medical specialties were conducted. Analysis identified ways in which self-tracking can support patient-provider encounters as well as informed the design of informatics tools for providers and patients to negotiate and align knowledge of disease, goals, and expectations.

Learning Objective: Learn the approach, challenges, and opportunities for informatics solutions using patient-generated, self-tracked data by engaging with chronic disease management providers, to inform development of visualization tools to support shared decision making within the clinical encounter.
