National leaders recommend systematically documenting social determinants of health (SDH) in electronic health records (EHRs). Two years after an EHR-based SDH documentation tool was activated in 354 community clinics, it had been used in 41% of the clinics. Tool use was higher in clinics that were not school-based, served more older or fewer Medicaid / uninsured patients, or had better-documented patient poverty data. SDH documentation efforts should consider clinic characteristics that affect this practice change.

Learning Objective: After participating in this session, the learner should be better able to understand the clinic-level factors that are associated with adoption of EHR-based documentation of patients’ social determinants of health.
