The hospital rules-based system (HRBS) is the first rules-based informatics program developed at Mayo Clinic in the 1980s in an effort to improve patient care. In 2015, Mayo Clinic announced the selection of Epic as its partner for a single, integrated electronic health record. We describe the conversion of Mayo’s legacy decision support systems into Epic’s antimicrobial stewardship module for eighty hospitals and clinics across four states.

Learning Objective: Understand the implementation efforts, challenges, and successes in converting a complex legacy decision support systems to a commercially available electronic health record vendor.
