We analyzed rates of duplicate laboratory tests over 3 years at a large healthcare delivery network overall and those associated with care transitions. A significant proportion of tests (14% to 37%) appeared to be duplicates. Duplication was up to 3-times higher after hospital admission than after discharge to community. We hypothesize that when hospitalists have a problematic access to outpatient records they order the tests they need even if they may be repeats.

Learning Objective: Readers will learn how duplicate testing may be more prevalent after a hospital admission when the history of outpatient laboratory results is poorly accessible or impossible.
