Description

Allery and adverse drug reaction documentation is poor in the current EHR system. There is no standardization of allergy documentation between institutions. Reaction type and criticality are left blank in over ½ of allergy entries, and 1/6 of reactions are entered as free-text. We propose a standardized enhanced allergy and adverse drug reaction list to assit better allergy documentation.

Learning Objective: Understand the importances of complete allergy documenation and the challenges associated with this.
