Description

Bridging the gap between research findings and what is available to patients in clinical settings has become a national priority. This mixed methods study used the RE-AIM (Reach, Effectiveness, Adoption, Implementation, and Maintenance) framework for clinical informatics to investigate the specific implementation factors necessary for real world adoption of a novel web-based diabetes self-management technology intervention, Mobile Diabetes Deective (MoDD), in four Federally Qualified Health Centers (FQHCs) serving low income and racial/ethnic minority adults.

Learning Objective: Identify key considerations when assessing translational potential of clinical informatics interventions using the RE-AIM framework.
