Although alerts in the electronic health record are an important safety tool, they are often ignored by users. Many studies of user response to alerting have focused on medication alerts, but little examination has been conducted of custom alerts created by institutions to meet their clinical decision support needs. We aimed to characterize the rate at which custom alerts are seen and user responses to these alerts at our institution. We found that custom alerts are commonly seen, mostly by nurses and prescribers. Although most custom alerts have a high override rate, some are accepted frequently. Further study is needed to understand the characteristics of these alerts that drive user responses.

Learning Objective: Describe the recipients of custom alerts in the electronic health record and the variability in alert acceptance for custom alerts.
