Description

Improved care coordination improves patient outcomes and decreases overall cost of care. At Memorial Sloan Kettering’s Pediatric Ambulatory Care Center, we implemented two new digital care team tools for facilitating efficient communication and improving patient-centered care, including a secure text messaging platform and an EHR-based care team identification tool. System audit data collected after implementation shows successful adoption of both tools by the multidisciplinary teams.

Learning Objective: Learn about the challenges and opportunities associated with the implementation of care team communication and coordination tools in a multidisciplinary pediatric cancer care center.
