Description

This study describes our approach to developing an electronic chart review tool that can be used to identify and assess cases of diagnostic error in the acute care setting. The tool captures the number of diagnostic errors in an episode of care, the actual or potential harm caused by these errors, and specific failure points in the diagnostic process.

Learning Objective: Learn challenges and possible solutions to identifying and assessing cases of diagnostic error in the acute care setting.
