Interoperability has generally been discussed as a binary property. Two systems can either share structured data or not. We argue that there is a continuum of interoperability. We present de-identified data from implementations of five commercial EHRs at oncology practices. We determined the standards that were followed and the consistency of structured data if no standard was used. Except billing, few implementations used standards. Vendors varied with respect to the consistency of non-standard data.
