Description

Acute conditions are commonly seen in the primary care setting. An electronic follow-up survey was automatically sent to patients after an encounter for any of seventy acute conditions. After one month, provider engagement was 57% and patient engagement was 46%, indicating this is an effective method of follow-up. Preliminary logistic regression models show factors predictive of patient and provider engagement, indicating specific areas of further study.

Learning Objective: Understand the possible applications of an electronic follow-up survey for the management of acute conditions in the primary care setting.
