Description

For many small providers, integrating and visualizing information from multiple sources within their EHR could be difficult given limited training and financial resources. We have created a desktop-based patient history visualizer that takes in patient C-CDA files and easily produces a visualizer for measuring adherence to evidence-based guidelines on co-morbid diabetes and hypertension. This visualizer will be piloted within a group of eligible small providers and the usage and effectiveness of this tool will be evaluated.

Learning Objective: After participating in this session, the learner should be better able to:
1. Understand the challenges faced by small providers in using their EHRs, especially in retriving information from different sources within EHR
2. Understand how patient histories could be composed from EHRs, and the best practices to demonstrate such information to physicians
3. Understand the tools for reading C-CDA files and the relevant information it contains. This information could then be used to formulate other uses for data that is contained in C-CDA files
4. Learn about how physicians take advantage of evidence-based care and how building on EHRs can help phyiscians make those decisions
