Description

Introduction: Diagnostic errors are an ongoing threat to patient safety.
Methods: We have identified symptoms and conditions in which diagnostic error may lead to harm in the inpatient setting. We are identifying and analyzing representative cases of these symptoms and conditions to determine root causes of diagnostic error.
Results: 1611 cases with possible diagnostic error have been identified.
Conclusion: Root causes of error will serve as potential targets for intervention.

Learning Objective: 1) Understand an approach for identifying representative cases of diagnostic error; 2) Apply the Safer Dx framework and DEER Taxonomy tool to identify root causes of diagnostic error.
